RemoveEmployeeFromAccountSetup Constructor

Username of the Employee to be removed from the Account.
Identifier for the Account the Employee will be removed from.
Employee Role to be removing from the Account
Initializes a new instance of the RemoveEmployeeFromAccountSetup class.
